Abstract Introduction Cavernous liver haemangiomas or hepatic are the most common benign neoplasm of liver. They more in women with a male to female ratio 2.5-5.1- likely due estrogenic influence. composed endothelial cells and usually small size (< 4 cm). asymptomatic cases incidentally discovered on diagnostic tests. Occasionally can exceed 5 cm where they cause symptoms such as nonspecific abdominal pain. Their exact aetiology is not well understood; thought be vascular malformations hormonal congenital background. Haemangiomas belong ‘Incidentaloma’ tumours, since found while investigating for other unrelated pathologies. Although rare, but rupture hemorrhage hemangiomas fatal. Case presentation A 33-year-old woman was presented pain which started central abdomen then shifted right lower abdomen. No significant at past medical history. On serial physical examination, patient tender quadrant her inflammatory markers were normal decision made proceed laparoscopy rule out acute appendicitis. During appendix multiple lesions. Appendix removed. Postoperatively CT Thorax, Abdomen Pelvis contrast Liver Triple Phase performed confirmed haemangioma. The recovered after surgery discharged home no routine surgical follow up.
